Qualifications

Prior to the performance of any work Sub-Contractors are required to supply the following
business and insurance information:

  1. W-9 Taxpayer Identification Number and Certification
  2. Current copy of Contractor’s or Business License
  3. Current business phone number and mailing address
  4. Business office Email address

General Liability

Workers Compensation

500,000 / 500,000 / 500,000

*Insurance certificates must be updated annually
*WestScott Construction, Inc. must be listed as a certificate holder

WestScott Construction, Inc. requires all subcontractors to use the pay application for
payment

Please note that under no circumstances will payment be issued until all qualifications/insurance are met.

Less 10% Retainage

Retainage = 10% of the sub total for the draw requested
